Thank you for your interest in a consultation with Legacy Lawyers, your estate planning, elder law, and asset protection law firm.  We look forward to working with you to achieve your estate and wealth protection planning goals.

We want to make this appointment as productive as possible.  To most effectively use our time together, we ask you to fill out our Estate Planning Questionnaire in its entirety. Completing the questionnaire will allow us to use this meeting to concentrate on the issues you consider most important to you.  

All information given to our firm in the questionnaire is subject to attorney-client privilege and will be kept confidential.

Please fill out the questionnaire in ink and verify all name spellings, addresses, phone numbers, etc. and adhere to the following guidelines:

  1. If you are married, both you and your spouse must attend the initial consultation.  If for any reason your spouse is unable to attend initial consultation, please call and let us know in advance.
  2. If you are requesting an initial consultation for someone other than yourself, perhaps a parent or other loved one, please fill out the questionnaire with their information.  If you are acting under a Durable Power of Attorney, please provide a copy along with the questionnaire.  
  3. All decision-makers must attend this initial consultation. For example, if you are acting under the power of attorney for a parent and need agreement by your siblings to make decisions in planning for your parent, the siblings, or decision-makers, must be in attendance either in person or by phone.
  4. If you need more room than what’s provided in the questionnaire, please attach extra pages.

If you have prior estate planning documents, such as a Last Will & Testament, General Power of Attorney, Health Care Power of Attorney, Trust, LLC, Corporation, or other important legal documents, please bring copies of those documents with you to your scheduled appointment.
